Job description/Responsibilities: 
Prepares, compound and dispense medications and pharmaceuticals to patients and employees of the hospital according to written and/or verbal instructions and standard pharmacy procedures. Reviews all medication orders fro inpatients prior to administration. Confers with and assist nurses with interpretation of physician medication orders, answering questions concerning drug and their administration as required. Participates and consults with physicians as required regarding medications and providing advice and information on drugs, their dosage forms, strengths, incompatibilities, toxicity and adverse effects. Supplies accurate drug information to all medical and paramedical personnel as required including drug identification and poison information. Formulates, prepares and dispenses preparations and special dosage forms not commercially available on retail market as necessary. Maintains overall control on quality of all drugs dispensed and prepares all labels properly for all packages containing drugs. Prepares and dispenses narcotics and all controlled substances according to Federal and State regulations. Maintains all related records as required. Maintains quality controls and records on investigational drugs as required. Prepares and maintains patient drug profiles as required to prevent drug incompatibilities and duplication of therapeutically similar drugs, and generally prevent medical problems caused by drugs. Reconstitutes and dispenses various drugs for enteral, parenteral, hyperalimentation, and irrigating use as required. Confers and answers questions concerning IV additives and their incompatibilities and mode of administration as necessary. Dispenses medications, specialty products, chemicals, etc., to all ancillary services. Delivers patient medication to nursing units according to schedule. Performs daily checks of patient care areas to assure proper storage, labeling and conditions of stocked drugs. Gives special attention to shelf life. Inspects and checks drugs stocked in emergency kits as required. Performs checks on night medications dispensed after pharmacy hours and restocks night medication cabinet as required. Maintains a state of readiness for all disaster situations as directed. Checks, instructs and assists all other Pharmacy personnel as required. Maintains awareness of current developments of drugs and procedures. Assists in maintenance for all records and controlled procedures as required such as narcotics, alcohol, controlled substances, investigational material and assists in physical inventory as required. Detects and reports improper operation, faulty equipment, defective materials, and unusual conditions to proper supervision. Maintains work area and equipment as required. Monitors patient drug regimens, including per pharmacy regulated therapies per protocol and drug regimen reviews. (The medications approved for per pharmacy regulation are: Aminoglycosides, Aminophylline and Vancomycin). Performs other duties as assigned or required.

Candidate Profile
1. Current Ohio state Registered Pharmacist License.
2. Experience in hospital pharmacy operations preferred.
3. Post Baccalaureate education in hospital pharmacy practice preferred. e.g., Residency programs for senior staff.
4. Starting in January 2013, all new hires require Bachelors Degree, preferably in pharmacy or related field.
