Online Applications are invited from the eligible candidates for the engagement of various posts in Jalpaiguri District under XV-FC HG, NHM, NUHM & AYUSH programmes as given below on contractual / Temporary basis from 12.05.2023 at 10:00 AM to 26.05.2023 at 11:59 PM for Jalpaiguri District.

Eligibility
Essential Criteria : Minimum Qualification
1. The candidate should have 02 (Two) years Diploma in pharmacy (D-Pharma) (Allopathic) recognized by the Govt, of West Bengal and registered as ‘A’ Category Pharmacist under West Bengal Pharmacy Council.
2. Weightage will be given for higher qualification.
3. The candidates must have proficiency in Bengali and must have efficiency in computers including MS Office & Internet.
4. The applicant must be permanent resident of West Bengal.

Age as on 01 01.2023 : Min. 18 Yrs to Max. 40 Yrs

Remuneration (In Rs) : Rs. 22000/- per month (Consolidated)

Place of Posting : UPHCs under Municipalities (Jalpaiguri or Dhupguri)

• Qualification (D- Pharma / B- Pharma /M Pharma) Max. 85 Marks (proportionate marking) (For D-Pharma 65 Marks / For B- Pharma 75 Marks / For M- Pharma 85 Marks)- proportionate marking
• Computer Test 15 Marks (Qualifying Marks Minimum 50%)

3. Original & Self Attested Photo copy of Admit card of Class-X.
4. Original & Self Attested Photo copies of Mark Sheets of Class-X, Class-XII, Graduation, Post graduation, MBBS/DNB/HMO/SAMO/UMO/GNM/B.Sc(Nursing)/ANM/DMLT/DLT (as per post criteria).
5. Original &SeIf Attested Photo copy of Caste certificate (to whom applicable)
6. Original &Self Attested Photo copy of Diploma / Certificate course in Computer Application (as per post criteria).
7. Original & Self Attested Photo copy of Registration Certificate of MBBS/ DNB/HMO/SAMO/UMO/GNM/B.Sc (Nursing)/ANM/DMLT/DLT.
8. Original &Self Attested Photo copy of Experience Certificate(Experience certificates must consist of Name of the post, Employer’s Name, Employee’s Name, Date of Joining (DOJ) and Date of Leaving (DOL) otherwise experience certificates will be treated as invalid).
9. Experience Certificate must be produce in the official Letter Pad of the organization/lnstitution/Laboratory with mentioning the Clinical Establishment (CE) Registration Number (CE Reg. no. require only for pvt. organization/institution) otherwise experience certificates will be treated as invalid.
10. Candidates working in Government Organization must route their application through proper channel if their departmental rules requires so and produce NOC at the time of Documents Verification.
11. Original &Self Attested Photo copy of Voter card / Aadhaar card / other address proof.
12. Attested (by the Gazette Officer or Gr-A Officer of State Government) Photocopy of Voter ID Card / Ration Card for the post of CHA only.

Important Information for the candidate
1. The applicant must be an Indian Citizen.
2. No. of Vacancy may be changed in future.
3. Application Fees : For UR Rs.100/- only & For SC, ST, OBC (A/B) Rs.50/- only which is non refundable
4. The benefits of upper age concession up to 5 years are admissible only to SC & ST Candidates of West Bengal and up to 3 years to OBC-A & OBC-B Candidates of West Bengal. SC, ST & OBC Candidates of other states may apply for UR vacancy as General Candidates.
5. In case OBC candidates category ‘A’ or “B” must be mentioned specifically in the caste certificate otherwise the candidate will be treated in “Unreserved” category.
6. One candidate may apply for multiple posts if he/ she is eligible.
7. Multiple applications for single post liable to cancellation of candidature.
8. The selection committee reserves the right to reject any online application which is /are not properly filled up.
9. Failure to submit any requisite documents liable to cancellation of candidature at the time of physical verification.
10. No TA/DA will be paid to the candidates for the selection test / interview

13. The engagement of above-mentioned posts are purely contractual / Temporary basis.
14. The service may also be terminated by one (01) month's notice from either side.
15. Exact date, time & venue for Documents Verification/Computer Test / Interview will be published in the official website of DH&FW Samiti, Jalpaiguri- jalpaiguriheaith.com/Recruitment.
16. All types of correspondences related will be done by online procedure only viz. Online notice board, exclusive candidate login portal
17. If the aggregate mark after final selection is equal then preference is to be given to the candidate inter se-senior in age as on 01.01.2023.
18. Any omission/suppression of information shall lead to rejection of application or candidature at any stage of the process without further intimation. The conditions so prescribed shall not be relaxed.
19. Candidates are instructed to not send (postal/ courier/ by hand) hard copy of application form & other relevant documents at this end.
20. All hard copies along with filled up online application form in original & photocopies of relevant documents is to be submitted at the time of physical verification after intimation from this end.
