Job Summary:
Responsible for a wide range of clinical functions, including review of appropriate drug therapy, calculations in evaluating serum drug concentrations in relation to pharmaceuticals, dose and inter-patient variables in select pharmacotherapy patients. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Job Responsibilities:

• Under the direction of the Preceptor, coordinates and provides clinical pharmacy services for patients receiving treatment. Identifies service requirements by interaction and consultation with patients, physicians, nurses, pharmacists and technicians. Monitors for appropriateness of medication, dose, strength, expiration date, route of administration and dosage form are appropriate for each patient, as documented in patient medication profile. Patients receive the correct medications for the optimal length of time.
• Documents clinical interventions, which prevent or eliminate drug-related problems (e.g., adverse drug reactions, drug interactions, etc.), or promotes cost-effective drug therapy. Prepares and submits concise activity reports documenting the number of interventions, estimated cost savings and potential change in patient outcome. All intravenous medications are converted to the oral route whenever possible and appropriate for the patient. Nursing is notified of any discrepancies. Provides specialty consults as assigned in a timely, accurate and consistent manner. Calculates pharmacokinetic parameters and communicates recommendations. Appropriately intervenes for drug allergies, potential drug interactions, and duplication of therapy. Serum drug concentrations are recommended, obtained and evaluated when consulted. Optimal dose adjustments based on therapeutic drug monitoring results and/or the patient’s clinical status are recommended and implemented in a timely manner. Non- consulted patients’ levels are also reviewed and recommendations are made to the prescriber when needed.
• Drug information and recommendations are provided to physicians, nurses and other healthcare professionals. Effectively communicates to medical staff recommendations for alternative drug regimens for all drugs which are not on the hospital formulary. Educates and counsels patients on how to take their medications properly. Patients receive appropriate information regarding possible side effects. The resident attends patient care rounds and actively participates as a member of his/her assigned medical team routinely as requested by the preceptor.
• Participates in education presentations to healthcare personnel on the principles of pharmacokinetics and/or pharmacotherapy as needed. Prepares drug monographs for review by the Pharmacy and Therapeutics, Formulary and MUE Committees as needed.
• Coordinates and participates actively in staff development programs (i.e., presents interesting cases or reviews journal articles). Nursing staff and other health care professionals receive timely and effective in-service regarding new procedures or pertinent new drug therapies (may include newsletters, etc.). A resource for drug information for other healthcare workers.
• Participates as a preceptor for Pharm.D. students as requested by the preceptor.
• Handles, processes and appropriately disposes of all waste materials, including all levels of hazardous waste, within the department. Appropriately uses Personal Protective Equipment (PPE) and Chemo Spill Kits in the event of a cytotoxic spill.
• Receives appropriate training in the handling of hazardous waste during initial training into the department, and then receives an annual update thereafter.
Knowledge, Skills, Education, & Experience Required:
• Graduate of a Doctor of Pharmacy degree program
• Good communication skills, ability to work with staff members
• Ability to work closely with medical, nursing and pharmacy services in the area of pharmacotherapy (Preferred)
• Ability to work with pharmacy and medical students when appropriate (Preferred)
• Ability to take active part in pharmacy staff development (Preferred)
• Ability to serve the needs of neonatal, pediatric, adult, adolescent and geriatric patients (Preferred)
