Even though the state of Kerala has advanced as compared to the other states of India in terms of critical health indicators are concerned, the state is facing challenges that are unique and specific. The people are now facing the problem of high morbidity both from re-emergence of communicable diseases. The second generation problems like the ageing population and non communicable diseases. Moreover, there remains the challenge of sustaining the privileged health indicators. Further, improving the quality of health care where the health seeking behavior is very high is of utmost importance. The resources of National Rural Health Mission came in an opportune time when the state was finding it difficult to find resources matching the demand.

The NRHM is an effort to strengthen the hands of States to carry out the required reforms. The Mission would also provide additional resources to the States to enable them to meet the diverse health needs of the citizens. While recognizing the leadership role of the states in this regard, it is proposed to provide necessary flexibility to the States to take care of the local needs and socio-cultural variations. In turn, States will decentralize planning and implementation arrangements to ensure that need based and community owned District Health Action Plans become the basis for interventions in the health sector. The States would be urged to take up innovative schemes to deal with local issues. Keeping in view the decentralization envisaged under the NRHM, the States would be required to devolve sufficient administrative / financial powers to the PRIs.

The National Rural Health Mission (NRHM) was launched by the Honble Prime Minister on 12th April 2005, to provide accessible, affordable and quality health care to the rural population, especially the vulnerable groups. The Union Cabinet vide its decision dated 1st May 2013, has approved the launch of National Urban Health Mission (NUHM) as a Sub-mission of an over-arching National Health Mission (NHM), with National Rural Health Mission (NRHM) being the other Sub-mission of National Health Mission.

Health and Family Welfare programme is being implemented in the state to sustain the achievement of the previous years and to reach new goals. NRHM is working in a task mode to achieve goals set-in for the state and effectively deal with the challenges. NRHM has set the strategic direction that encompasses year wise objectives, technical strategies; inter-ventions include program and services for improving maternal health, child health, family planning, adolescents' health etc. RCH –II has been bifurcated into core programmatic strategies and cross cutting program strategies for taking effective actions. The institutional and other strategies have impact on all the components of RCH viz. maternal health, child health, family planning, adolescent health and sex selection etc whereas specific core program strategies have wider impact on the specific program component. It has been built that all these strategies would converge and go hand in hand to achieve the program outcome.
