Pharmacist {5 Posts}
PB-1:Pay Band
Corresponding Pay Bands: 5200-20200
Grade Pay: 2800
UR-04 OBC-01 [Total-05]
Qualification: Degree in Pharmacy QR Matriculation or equivalent qualification from a recognized board and qualified and registered compounder/pharmacist.
Age: Not exceeding 25 years. (Relaxable up to 35 years in case of Govt. servant and employees of the ESIC)
How to apply:-
01. The crucial date for reckoning the age and other eligibility criteria i.e. educational/technical qualification, experience, internship, registration, course etc. will be the last date of receipt of applications i.e. 28-06-2010.
02. The application in the prescribed application format given below duly filled in English or Hindi language, duly supported with clear / legible attested copies of the relevant certificates and marks statements (in English or Hindi) should be sent by Registered post / Speed post at the following address :-

03. Candidates have to fill in the application form in their own handwriting with blue or black ball point pen.
04. Origmal documents I certificates should not be sent with the application form.
05. Candidates should submit one application only for one post. The candidature of those candidates who submit more than onc application for one post shall be liable to be rejected.
06. Envelope conta in iug the application should be super scribed "Application for the post of ______ in ESr Super Speciality Hospital, Sanath Nagar, Andhra Pradesh.
07. Documents required to be attached with the application. Attested copies of certificates in support of
1) Date of birth
2) Educational & Technical qualification alongwith marks statement
3) caste / community certificate
4) Discharge certificate in respect of Ex. Servicemen.
5) Disability certificate in respect of PH candidates
6) Two recent passport size photograph duly attested by Gazetted officer. One photo should be firmly pasted (Not stapled or pined) in the space provided in the application form.
7) Two self addressed envelopes.
8) Demand Draft.
08. The Demand Draft should not be tagged or stapled, but should be pinned or clipped at the top of the application form. Candidates must write his / her name, address, Post applied and ESIC Hospital in which applied on the back side of the Demand Draft.
09. The Corporation reserves the right to call candidates for screening test or interview or both.
