The idea of establishing a Medical College in Shahdol, M.P. was conceived sometime back in 2015 as one of the projects for Government of India’s Pradhan Mantri Swasth Suraksha Yojana(PMSSY). District Hospital Shahdol was one among the seven places chosen for the upliftment where now stands a new Medical College. A formal administrative and financial approval was passed by the Pariyojana Samiksha Samitee of the state of M.P. in the year 2016.

1. Passed 12th examination in 10+2 system in Biology, Chemistry and Physics.
2. Diploma in Pharmacy.
3. Live Registration of Pharmacist from MP Pharmacy Council.
4. Working experience of 05 years in the post of Pharmacist Grade-II
Salary : 5200-20200 + 2800 (Level-7 of 7th Pay Scale, Minimum Pay Rs.28700/-)

Pharmacist Grade - II
No of Posts : 13
Qualification :
1. Passed 12th examination in 10+2 system in Biology, Chemistry and Physics.
2. Diploma in Pharmacy.
3. Live registration of Pharmacist from MP Pharmacy Council.
Salary : 5200-20200 + 2800 (Level-6 of 7th Pay Scale, Minimum Pay Rs.25,300/-)
Age Limit : 18-62 years
Application Fee :
For Unreserved Category Rs. 1100/- (Rupees one thousand one hundred only), Other Backward Classes (Only Non-Creamy Layer) and Economically Weaker Section Rs. 1000 / - (Rupees one thousand only), and for Scheduled Castes and Scheduled Tribes Rs. 900 / - (Rupees nine hundred only), the amount has to be deposited online.
Note
1. It will be mandatory to bring the original photo identity card in the examinations. The candidate can choose any one of Voter ID, Pan Card, Aadhar Card, Driving License and Passport as the original photo identity card. UIDAI The e-Aadhaar will be valid only if it is verified by UIDAI. In the absence of the original photo ID, the candidate will be debarred from appearing in the examination.
2. In addition to the original identity card, it is mandatory for the candidate to bring the information of his Aadhar card / e-Aadhaar card / photocopy of Aadhar card / Aadhar number / Aadhar VID. Multilevel biometric verification is mandatory at the time of admission to the examination and during the examination.
3. Candidates will be allowed to enter the examination center till the time of reporting in the examination. The candidates appearing for this will not be eligible for admission.
4. The use of electronic devices such as mobile phones, calculators, log tables and copy sheets etc. is strictly prohibited in the examination hall. 5. Candidates can get their admit card for the online examination only through the online application form number. Therefore, it is necessary to keep the application number number with you, for which all the responsibility will be of the applicant.
6. At the examination center, the applicant should have black ball point pen and MP for admission in the examination hall. It is mandatory to bring the admit card downloaded from the online portal.
7. The medium of examination will be English (English) Hindi.
8. No candidate will be allowed to leave the examination hall after the commencement of the examination till the end of the examination.
9. At the time of filling the application form, any of the certificates of the candidates will be tested by M.P. will not be done online. The original documents of the candidates will be verified by the institution. Therefore, the eligibility of the candidates in the computer based online examination will be completely provisional.
10. It will be mandatory for the candidates to have live registration in the employment office of Madhya Pradesh state.

3. The following scanned copies of documents are mandatory to be uploaded by the candidate along with the application:
• High School Examination Marksheet / Certificate / Birth Certificate in support of the date of birth.
• 12th class mark sheet.
• Marksheet of the respective degree / diploma / final year in support of the qualification prescribed for the post applied for.
• Living registration certificate from the concerned council in Madhya Pradesh.
• Photo ID card.
• New photograph and signature of the candidate.
• Caste certificate .
