Job Description
1. Provide efficient and high quality training and support to existing dialysis patients by visiting them at home. The Patient follow up by the Clinical Coordinator will include the following:
2.Every patient should have a baseline report filled immediately after catheter insertion
* Every patient will be visited by a Clinical Coordinator at least once a month on topmost priority.
* The Clinical Coordinator will diligently fill the patient follow up sheet after probing the patients for their well being. The Clinical Coordinator will examine the patient for their physical signs and symptoms of malnutrition, fluid overload, anaemia, level of activity, depression and record them in the follow up sheet. The Clinical Coordinator will use the HRA chart to assess the risk status of the patient.
* The Clinical Coordinator will be responsible for monitoring bag sale index and assess the patients for their compliance on the therapy.
* The Clinical Coordinator will consult the Clinical Team Leader or the Area Clinical Coordinator and the Nephrologist in managing complications.
* The Clinical Coordinator reports patient coverage and losses in their weekly report every Monday to the Clinical Team Leader and the KAM. Clinical Coordinators are accountable for loss reporting.
* Patient coverage, Transfer set exchange are reviewed by the ACC,KAM in their weekly reviews with Clinical Coordinators. Shortfall in patient coverage and transfer set exchange are identified and alternate plans made to achieve complete patient coverage.
* Each patient’s baseline and follow up sheets will be discussed with the treating Nephrologist once a month by the Clinical Coordinator.
3. Achieve targeted improvement in therapy outcome (surviving number of patients) by providing clinical support to existing patients. The Clinical Coordinator provides feedback to the patient’s nephrologist on their peritoneal dialysis practices and state of general well being.
4. Achieve initiation targets by educating and counseling prospective patients and providing nephrologist with clinical support for patient initiation on continuous ambulatory peritoneal dialysis or automated peritoneal dialysis.
5. Help maintain clinical database of existing and new patients on CAPD.
6. Educate nurses and technicians on Baxter products and services for CAPD propagation and enhancing improved PD outcomes.
7. Conduct scientific meetings and organise get together for the patients and doctors in consultation with the key nephrologist.
8. Attending scientific meetings and presenting papers at various scientific forums to keep themselves updated on the recent advances in the therapy in consultation with their respective nephrologist and corporate clinical heads.
9. Monitor competitor activities related to patient initiation and promotional activities and provide feedback to sales field force(TM/KAM/ASM/SM/NSM).
10. The Clinical Coordinators have a direct reporting to the Clinical Team Leaders or Area Clinical Coordinators of their region.
Candidate Profile
The Clinical Coordinator is a fresh graduate from bachelor of Nursing, diploma of nursing, bachelor of Science/biology or a person with 1-3 years experience in Nursing, Hemodialysis or CAPD who is willing to contribute a helping hand to critically ill renal disease patients & their family. Practical knowledge of computer application (MS office-word, power point & excel sheet, and internet surfing) is an advantage.
