Job Description :
• Before Administration, Medication review and clinical audits on the floor.
• Transcription / Indenting of drug and non-drug orders.
• Responsible for medication tracking and receiving from pharmacy.
• Auditing all the in-patient orders for drug name, strength, formulation, dosage and duration of therapy, therapeutic duplication and to check for possible drug interaction, if any as per hospital protocols.
• Checking the Medication Administration Records in the ward and rectification of any medication error observed.
• For clarification check the patient record at the ward / ICU discuss with Clinical Pharmacologist.
• In case any non-formulary drugs are indented inform to the concerned doctor and substitute it with the drugs given in the formulary.
• Responsible for checking the drugs in impress stock, crash cart, disaster kits and ambulance regularly.
• Responsible for documentation of restricted antibiotics and report it to Clinical Pharmacologist.
• Monitoring of adverse drug reaction.
• To check for Prescription, Indent and Audit medication error such as correct dose,formulation, route, strength and frequency.
• To inspect for Therapeutic duplication.
• To examine drug interaction among the patients. To examine food versus drug interaction among the patients.

• To check the home medication and registering them in a prescribed format with the approved consent from treating consultant
• To respond all Drugs related query from Health Care Professionals. It is must to monitor ADR (Adverse Drug Reaction) and documenting the same.
• To conduct In-Patient ward rounds for checking the Drug charts and discharge summary.
• To detect medication errors and daily narcotic indent verification
• To Prepare the Chemo Admixture whenever required according to the defined protocols.
• To Prepare and reporting High-end Antibiotics protocols on daily basis.
• To report Food and Drug interaction status on a daily basis.
• To report antibiotic consumption on a monthly basis.
• Shall have periodical rotational duty at OP Pharmacy and responsible for prescription auditing and patient counseling.
• It must to follow guidelines in Storage and handling of narcotic drugs.
• It is essential to make rounds along with Consultant or Doctor upon their preference.
• To maintain ethical practices and diplomacy at all levels that we are dealing in our day to day transactions.

Candidate Profile
Candidate should be from Pharma.D Background.
