Age limit
The candidate must have attained the age of 21 years and must not be above the age of 32 years as on 15th July 2015.The upper age limit shall be relaxed by 5 years in case of SC. ST, SEBC & Women candidates 10 yrs in case of physically handicapped candidates.
Further that the in sen ice pharmacists engaged on contractual basis under the central scheme, state plan scheme and externally aided schemes who are below the age of 45 years and have completed at least one yeai of continuous service shall be allowed to take part in the recruitment process as per carrier assessment.
Essential Qualification: The candidates must have passed +2 Science Examination under Council of Higher Secondary Education, Odisha equivalent and Diploma in Pharmacy from any of the three Medical Colleges and Hospitals of the state any other recognized private institutions duly approved by A.I.C.T.E. and examination conducted by the Odisha Pharmacy Board.
The candidate must satisfy the following condition
(i) The candidate must he a citizen of India.
(ii) Shall posses requisite qualification (age eligibility & educational) show at Para 2(A) & 2(B) above.
(iii) Be able to read and write and speak Odia: and have
(a) Passed middle school examination with Odia as language subject; or
(b) Passed Matriculation or equivalent examination with Odia as medium of examination in non- language subject; or
(c) Passed in Odia as language subject in the final examination of class- VII from a school or educational institution recognized by the Govt. of Odisha or the Central Govt.; or
(d) Passed a test in Odia in Middle English School standard conducted by the School and Mass Education Department.
(iv) Be of good mental condition and bodily health and free from any physical defect likely to interfere with the discharge of his duties in the service.
(v) If married must not have more than one spouse living.

2. Attested copy of HSC or equivalent Certificate & Mark Sheet.
3. Attested copies of 1 2 Science or equivalent Certificate & Mark Sheet.
4. Attested copies of Diploma in Pharmacy Certificate & Mark Sheet.
5. Attested copy of the recent C aste cert ificate issued by the competent Revenue Authority.
6. Attested copy of the recent (issued within 6 months to the date of advertisement) Residence certificate issued by the competent Revenue Authority.
7. Attested Cop> of valid Employment Registration Certificate.
8. One self addressed envelope (size 24" X 10") with postage stamp of Rs.25/- .
