Applications are invited from the eligible candidates in the prescribed form for engagement of 10 nos of Pharmacist on contractual basis on a monthly remuneration of Rs. 5200/- plus Grade Pay Rs. 2400/- for Drug Distribution Centres under Chief District Medical Officer, jagatsinghpur. The applications should reach the office of the Chief District Medical Officer, Jagatsinghpur on or before 29.07.2015 by 5.00 PM only by Registered Post/Speed Post. The sealed envelope containing the documents should be superscribed "APPLICATION FOR THE POST OF PHARMACIST (DDC)". The application received after due date and incomplete manner will not be entertained and liable for rejection. Preference will be given to the own District candidates. In case of non availability of eligible own District candidates, other district candidates will be considered. ORV Act & rules is applicable for the post. Nationality: She/ He must be a citizen of India
Qualification: Must have passed +2 Science Examination under CHSE Odisha/equivalent and Diploma in Pharmacy from any of the 3(three) Medical College & Hospital of the State/any other recognized private Institutions duly approved by A.I.C.T.E and examination conducted by the Odisha Pharmacy Board.
Age Limit: She/He must have attained the age of 21 years and must not be above the age of 32 years on the date of advertisement. The upper age limit shall be relaxed by 5years in case of SC, ST, SEBC & Women candidates and 10 years in case of Physically handicapped/Ex-Serviceman Candidates.
Knowledge in Odia: The candidate must be able to read, write and speak odia and have passed in odia as per rule.
Nature of Engagement: Contractual for one year to be renewed on satisfactory performance and subject to signing of agreement as prescribed by Govt, of Odisha, Finance Department.
ORV Act and Rules is applicable for this contractual recruitment as per existing guideline.
Marital Status: If married, the candidate must not have more than one spouse living.
Physical Fitness: The candidate must be of good mental and physical health and free from any physical defects likely to make him incapable of discharging her/his normal duties in the service.
The post is purely temporary which can be terminated at any time without assigning any reason thereof.

The candidates, who are already working in Health & FW Department on contractual basis, have to apply through proper channel.
The Pharmacist engaged on contractual basis under the Central Plan, State Plan schemes and externally aided Scheme who are below the age of 45 years as on the date of advertisement and have completed at least one year of continuous service can apply through proper channel. Such candidates will be allowed one percent extra mark of the total marks for each completed year of continuous service subject to a maximum of fifteen percent which will be added to the marks secured by them for deciding the merit position.
The candidates are required to enclose the following documents along with the application form duly attested/self attested;
i) Attested Xerox copy of HSC Examination along with mark sheet
ii) Attested Xerox copy of CHSE Examination along with mark sheet
iii) Attested Xerox copy of Diploma in Pharmacy pass certificate along with mark sheet
iv) Attested Xerox copy of Registration Certificate of Odisha Pharmacy Council
v) Attested Xerox copy of residence certificate from competent authority valid on the date of advertisement.
vi) Attested Xerox copy of the caste certificate from competent authority
vii) Two recent passport size photograph duly attested by Gazetted officer
