Notice regarding application
1) All candidates are required to fill up the application form in the online Google form on the Google link mentioned in the advertisement within the prescribed period. ( forms.gle/ZFr42vzkawxdQ7TPA for district level posts) (for state level posts - forms.gle/3yMqefzuxPJrWQTr8)
2) It should be noted that once filled in the information will be considered final and no changes can be made in it.
3) After completing the Google Form, print it out.
4) For the information mentioned in the above print and application along with attested copy of required certificates / documents in the application point no. 10 should be submitted to the address mentioned in the prescribed time limit. Applications received after a given period will not be considered. It should be noted that if the application received by post / courier is received late, it will not be considered.
5) Do not attach unnecessary documents with the application.
6) If the applicant does not attach a copy of the Google Form filled in with the required certificate / document attached with it, such application will be deemed incomplete and it will be canceled.
7) Candidate is required to fill only one Google form for each post.
8) Candidate can apply for more than one post. However, a separate Google Form is required for each post and it is mandatory to submit it to the office within the stipulated time as per the above instructions. It is necessary to add a separate check of application fee for each post.
Age Limit
1) The last date for filing Google application i.e. 26th February 2022, the age of the candidate should be within the age limit prescribed in the advertisement for each post.
2) The maximum age limit for candidates in the reserved category will be relaxed after five years.
3) At present the maximum age limit will not be applicable to the employees working on contract basis in the National Health Mission. However, it is mandatory for the head of the office currently working with the application to submit a certificate to that effect.
Instructions for filling the application
1) First select the name of the post for which you want to apply from the drop box (DROP BOX).
2) Candidate should register his / her full name (Secondary School Examination (SSC) Certificate) as accurately as possible. A copy of Secondary School Examination (SSC) Certificate should be attached with the application.
3) If the applicant is married then marriage registration certificate and name Gazette must be submitted along with the application in case of change.
4) The date of birth mentioned in the Secondary School Examination (SSC) certificate should be mentioned in the application.
5) The age (year, month and day) of the last date for submission of Google form (February 26, 2022) should be mentioned in the application.
6) Gender, marital status should be mentioned in the application.
7) It is mandatory for the candidate to mention his / her valid email id / alternate email id, current mobile number / optional mobile number in the application. Eligible candidates for the interview will be notified in the eligible / ineligible list as per the requirements - the instructions regarding the interview will be given by email and the list of eligible candidates for the interview will also be published on the website. Therefore, the email ID will continue till the recruitment process is completed and it will be the responsibility of the candidate to check the email from time to time. It is also mandatory to visit the website from time to time for the list of eligible / ineligible candidates for recruitment and necessary instructions and corrections regarding recruitment will be published on the website of this office (arogya.maharashtra.gov.in) from time to time.
8) It is necessary to specify whether the candidate is a resident of the State of Maharashtra or how. If so, the prescribed certificate (Domicile Certificate) should be submitted along with the application.
9) Candidate should mention his religion and caste details accurately. If the candidate falls in the reserved category, the prescribed certificate should be submitted. If the candidate in the reserved category does not submit the caste certificate, such candidate will be considered as a candidate in the reserved category and will not be able to avail the reservation and other benefits available thereon.
10) Candidates belonging to Other Backward Classes, Deprived Castes and Nomadic Tribes, Special Backward Classes, Economically Weaker Section will be required to submit the required non-creamy layer certificate for the current year. Otherwise the applications of such candidates will be rejected.
11) The applicant should specify his current address and permanent address in Google form.
Educational Qualification / Eligibility
1) Detailed and accurate details regarding educational qualification should be recorded.
2) At the last date of submission of Google application, the candidate should have the minimum educational qualification required for the post.
3) The date on which the required educational qualification is obtained must be mentioned in the application. (Date of declaration of results)
4) While filling the application, the percentage of marks obtained in the final year should be mentioned accurately. Percentage of marks should be mentioned without mentioning grade or other category. If the final certificate mentions a grade or grades, it should be certified by the concerned institution by converting it into marks. And submit a self attested copy.
5) Exact percentage of marks should be mentioned in the application without mentioning the total marks obtained. If the percentage of marks mentioned in the academic certificate and the percentage mentioned in the application do not match, such applications will be rejected.
6) It will be mandatory to submit all the certificates / documents related to all the educational qualifications mentioned in the application along with the application. The application will not be considered unless the required academic qualification marks are added during the scrutiny of the application.
7) The required educational qualification for the post should be obtained from a government recognized university. Academic qualification / qualification obtained from the universities mentioned in the Government Decision dated 12 December 2006 will be considered.
8) At the last date of submission of Google application, the candidate should have the minimum educational qualification required for the post.
Experience
1) Experience will be accepted only after obtaining the prescribed educational qualification and additional required educational qualification for the post being applied for. Accordingly, experience prior to holding educational qualification should not be recorded. It will not be noticed.
2) When mentioning the details of experience, the first appointment period to the current appointment period should be given in this order.
3) While stating the details of experience, the details of the office from which the experience certificate is obtained should be mentioned in the form. Without experience certificate, this experience will not be considered. The duration of experience should be clearly stated in the experience certificate.
4) When mentioning the details of experience, the date of commencement and the date of dismissal should be mentioned. Care should be taken to record both the dates as per the experience certificate. Such an experience will not be considered if there is a difference.
5) Only the experience required for the post for which you have applied will be considered. An experience other than this will not be considered.
Application Fee
1) Candidate is required to attach the sum of application fee along with the application. Demand Draft application fee of Rs.200 / - for open category candidates and Rs.100 / - for reserved category candidates. The following names should be drawn. "STATE HEALTH SOCIETY MAHARASHTRA SNA"
2) Applications received without proper withdrawal will not be considered.
3) If you are applying for more than one post, add separate funds for each post.
Candidate's photo and signature
Do not staple photos. Sign in the space reserved for signature.
The last date to fill out the Google Form is February 26, 2022 at 11.59pm.
After filling up the Google form, print it out. Attach the required certificate and self attested copy of the documents along with the printout and the application to the following address. Handwriting should be submitted before March 07, 2022 or it should be sent by post / courier till 6.15 pm during office hours.
