Age Limit
1. The minimum age for contractual recruitment will be 18 years and maximum 70 years for medical posts and 64 years for managerial posts. 2. Calculation of age will be done as per contract year date 01 January 2021.
Contract period and contract rules
1. The period of contract service will be from the date of appointment till 31 March 2022. Which can be determined every year by evaluating the annual work of the employees / officers, the continuation of the contractual service.
2. During the period of contract service, a lump sum honorarium or other amount will be payable as per the provisioned ROP. Apart from this, no other type of allowance / facility will be admissible.
Application Fees
Honorarium per month Rs 25000 or below Rs 25000 :

2. For payment of application fee, it will be mandatory to enclose a demand draft issued by any nationalized bank in the name of District Health Society, Raipur, which will not be refundable.
3. Interested candidates for recruitment to more than one post will have to submit separate application and prescribed fee (demand draft) for each post.
4. It will be mandatory to mention the number, date and amount of the demand draft in the application form.
5. Application without application fee (demand draft) will render the application invalid.
Application Guidelines
1. The application format of the applicant can be seen and downloaded from the website of Raipur district raipur.gov.in. The application will be accepted in the prescribed form only.
2. Applications in the prescribed format, completely filled in a closed envelope, will be accepted through registered post / speed post / registry only at the address of Chief Medical and Health Officer, District Raipur (Chhattisgarh) by 14.12. 2021 Will be done by 5:30 pm
3. It will be mandatory for the applicant to clearly mention the name of the post applied for, the name of the program and the cadre (UR, SC, ST, OBC) on the top of the envelope while applying.
4. Incomplete / unclear and erroneous applications will not be considered. Which will be the responsibility of the applicant.
5. The department will not be responsible for delay in receipt of application by the prescribed date after the issue of advertisement or for delay by post.
6. All the posts advertised under National Health Mission will be on contract basis.
Interested and eligible candidates may apply in the prescribed format to the Office of the Chief Medical and Health Officer, District Raipur, Chhattisgarh will be accepted by Registered Post, Speed Post and Registry up to 14.12.2021 till 05:30 PM during office hours.
