UK Health & Family Welfare Society was constituted in the year 2002 with aim to serve as umbrella society for all national programs and effect health sector reforms with the help of external funding agencies such as USAID.

Application are invited by District Health and Family welfare department under national health mission on contractual basis.
Qualification
B.Pharm / D.Pharm from any recognized institute and registration with state Pharmacy Council.
Salary : Rs. 15,000/-
Selection Process: Selection will be basis on Interview
Note:
1. The application form must be filled in A4 size paper. Otherwise, an application form will be rejected for which candidates will be responsible for themselves.
2. Send the application form in given format only. otherwise, the application will be rejected for which candidates will be responsible for themselves.
3. The application form will be rejected if it is incomplete for which candidates will be responsible for themselves.
4. In the case of incomplete filling of declaration form, the application will be rejected and for which candidates will be responsible for themselves.
5. In the case of nonavailability of self-attested photograph and self-attested educational certificates, the application will be recognized as incomplete and mark as rejected for which candidates will be responsible for themselves.
6. Candidates are requested to fill up their phone no/ email id in clear letters in the application form. Information regarding recruitment will be provided through email or phone and notice board of Office of the chief medical officer, Rudraprayag. In addition to these mediums, information regarding recruitment will not be provided through any other medium.
7. No. of posts may increase or decrease during the process of recruitment.
8. The recruitment process can be canceled without any prior information.
9. TA/DA will not be provided for the recruitment process.
10. Candidates who will be selected, have to work according to the rules of District Health and Family welfare department, Rudraprayag.

Eligible candidates should be applied in prescribed application format and sent it to
Office of Chief District Officer (Under NHM)
Rudraprayag, Uttarakhand, India
on or before August 14, 2017 along with attested educational certificates and recent passport size photograph.
Candidate must mention the valid e mail id, mobile number on the application form. Application will be accepted only by speed post or registered post.
Application form which will receive after last date, will not be accepted.
