Under National Health Mission, as per the approved ROP of the district in the year 2024-25, under National Health Mission / District Health Committee, District Kabirdham CG, applications in the prescribed form from interested eligible candidates for contractual recruitment on the vacant posts of MO Ayush RBSK Male, MO Ayush RBSK Female, 2nd Anm NHM, Nursing Officer-NMHP, Nursing Officer- NHM, Nursing Officer UHWC, Staff Nurse SNCU, Staff Nurse NBSU, Laboratory Technician BPHU, Pharmacist RBSK, Laboratory Technician RBSK, Community Health Officer (CHO), Psychologist Clinical, Radiographer - NHM, MPW Male - UHWC, STS (Senior Treatment Supervisor NTEP), Physiotherapist, Block Manager Account, Block supervisor -VBD should be sent to the Chief Medical and Health Officer, District Kabirdham CG.

Qualification : Degree or Diploma in Pharmacy (50%) & Registration in CG Pharmacy Registration council.
Salary : Rs. 16,500/-
Regarding application form / advertisement:-
1. This appointment will be subject to the final order of SLP (C) No. 19668/2022 of the Hon'ble Supreme Court of New Delhi.
2. The format of the application form for the advertised post and the general conditions and guidelines regarding the recruitment of the said posts and the list of selected candidates and other information related to recruitment can be viewed and downloaded from the district website kawardha.gov.in from time to time and information will not be given through any other medium. It will be the responsibility of the candidates to view it on time.
3. All self-attested documents along with the application form can be sent to the address of the Office of Chief Medical and Health Officer, District-Kabir Dham (CG) by registered post / speed post on 23/4/202.5 by 5:00 pm. Apart from this, applications will not be accepted through any other medium.
4. Applications received after the prescribed date and office hours will not be accepted.
5. The name of the post applied for and the cadre should be clearly written on the envelope of the application submitted for the advertised vacant post. In case the name of the post is not found mentioned on the envelope, the District Selection Committee will have the right to accept/reject the application.
6. The candidate must be a native of Chhattisgarh. It will be mandatory for the candidate to attach the domicile certificate issued by the competent authority.
7. Incomplete and erroneous applications will not be accepted. Every applicant should look at the instructions given in the advertisement and all the information in the application form and carefully fill the correct and complete information. If any information is given incomplete or erroneous in the application form, then the entire responsibility for it will be of the applicant. On the basis of error or incompleteness, the application form can be rejected at any stage of selection without prior notice to the applicant.
8. All educational documents for application to the above posts must be of the educational qualification prescribed by a recognized education board/institution/university and degree/diploma must be from a recognized university/institution. Computer related
9. It is mandatory to attach self-attested photocopy of complete mark sheet of each year of compulsory/desired educational qualification with the application.
10. Applicants can apply for more than one post but have to submit application and fee separately, if 02 or more applications are found in one envelope, only 01 application will be accepted. For which no claim made by the candidate will be valid.

• Application fee will be payable through demant draft in the name of "District Health Society Kawardha". Bank drafts made in any other name will not be accepted and such candidates will be disqualified for selection.
• Applications without application fee or application fee less than the prescribed amount will not be considered and the concerned candidate will be considered ineligible in the selection process.
3. Photocopy of bank slip, demand draft received from the bank as application fee will not be valid.
It is mandatory to attach all the desired documents with the original application and during the recruitment process, at the time of document verification, candidates must be present with the following documents:-
• 10th mark sheet.
• 12th mark sheet.
• Mark sheet of all the years of graduation and post graduation examination.
• Related degree.
• Computer diploma/degree mark sheet from a recognized institution.
• Registration certificate of the related council/board.
• Permanent caste certificate from the competent authority.
• Domicile certificate.
• Disability certificate (if any)
• Experience certificate (if any)
• No objection certificate from the current employer (if applicable)
• Identity card (Aadhaar/Voter card/PAN card/Driving license/other)
• Others (Candidates will attach self-attested desired documents with the application form as per the above order.)
Other terms and conditions
1. After receiving the appointment order by the contract employee, a contract has to be executed with the appointing authority on a non-judicial bond of Rs. 50/-. The expenditure incurred on the execution of the contract will be borne by the selected candidate. 2. It will be mandatory to execute an agreement letter (contract letter) with the employer.
3. It is mandatory to complete all the educational and other qualifications by the date of advertisement, otherwise the application form will be rejected immediately and placed in the category of ineligible and no correspondence will be accepted in this regard.
4. The prescribed format will be issued at the time of calling for claim objection for the claim objection application. Claim objection will be accepted only in the said prescribed format. No application will be accepted after the prescribed date for claim objection.
5. No new document of any kind will be accepted in the claim objection.
6. The process of resolving the claim objection will be done on the basis of the documents attached with the original application and only clerical errors will be corrected in the claim objection. Documents after the date of advertisement will not be accepted.
7. During the advertisement and recruitment process, factual/clerical/vacancies/typing etc. related errors can be corrected. Information of which will be duly published.
8. It will be mandatory to be present at the place of posting as per the appointment order. In case of not being present at the prescribed time and place of posting, the appointment of the selected candidates will be automatically cancelled.
Applications are invited through registered post / speed post in the name of till 05:30 PM on 23.04.2025, applications will not be accepted through any other medium. Applications received after the prescribed date and time will not be accepted under any circumstances.
